| Child must be at least 6 years old and have completed the 1st grade and no older than 12 years old by July 30 |
| Details: |
| Each child must have a current completed physical comleted by the doctor. Physical cannot be earlier than July 30, 2011. |

| Applications must be turned in by 06/15/12 |
| |
| If a child takes a orescription medication, the medication must be in the original container, enough for the entire week. Non-prescription medications are NOT allowed. |

Applications can be picked up at The Salvation Army Fredericksburg Office at 2012Lafayette Blvd, Fredericksburg, VA 22044 (next to Paul's Bakery).
