If the client is sent directly from court, what is the procedure once he/she gets to the probation office?
 
The client  is assigned a Probation Counselor in court by the Court Liaison.
 
How does the client set their next appointment?
 
The Probation Counselor sets up the next appointment. The client receives a printed statement of account that has all necessary information about their case including the next appointment. The client signs for this stating they received the statement and appointment.
 
How does the client cancel an appointment?
 

The client must speak with the Probation Counselor directly to re-schedule. Each Counselor has their own phone extension that the clients can call.

What should the client bring to the first appointment?
 

The client will need to bring their court papers, a photo ID, proof of residence, proof of employment, unemployment, disability or SSI (whichever applies) and their first month's minimum monthly payment in the form of cash or money order only.

If you are a juvenile and have been directed to The Salvation Army, you or your guardian should call the following number for more information: (904) 213-4881 and ask for Administrative Support, who will direct you to the correct person.

 
What forms will need to be completed at the first appointment?
 
This will vary by case. There are not any forms that can be pre-filled out at this time. All forms that the client needs will be given at their first appointment.
 
What services and classes are offered?
 

We offer different services. Call (904) 213-4881 for more information.  The Salvation Army provides a variety of services for those in need. For more information, click here. http://www.salvationarmyjax.org/

 
What is the process for community service work, insurance, etc.?
 
The client will pay $10.00 per every 50 hours of work they need to do. This cost is for the insurance. They will need to fill out a questionnaire form and they will be given a referral to an approved agency that the Probation Counselor and the client have agreed upon. We do this for non-probation clients as well and they pay a fee of $60.00 to cover the cost of the insurance.
 
What about referrals?
 
On occasion, in order to address a specific need, The Salvation Army will refer you to another community agency. Your counselor can provide you with a list of local providers. Call (904) 213-4881 for assistance.
 
What are the client's rights and responsibilities?

Be provided confidentiality with regard to HIPAA compliance
Be provided clear information regarding the conditions of your probation

Client Responsibilities 
Meet all conditions of probation as assigned by the court 
Keep all appointments
Be honest and open with your counselor
Pay your fees/fines according to the conditions of your probation 
Show respect and concern for other clients of The Salvation Army and to respect their privacy 
Show respect and consideration for the staff of The Salvation Army
 
Additional Information:
Closed on Mondays. Closed daily for lunch from 12 noon to 1:00pm. We see our last client at 5:00pm and we are closed the first Wednesday of every month as well as most major holidays.
