Transitional Housing Program


The Transitional Housing Program is a one month to two year program that provides housing and support services to individuals desiring to rise above poverty and to eliminate the constant threat of homelessness. Trained case managers help individuals and families work toward key objectives that allow them to become self-sufficient during this time. There are currently 55 beds offered through this program: nine beds for men, three beds for women and forty three beds for families. In 2012, an additional 34 beds will be offered in duplex units.

While in the program, families are provided with basic needs including shelter, food and clothing. Recipients of this program are often single parents who are trying to support two to three children. Targeted objectives for this program include: gaining employment, securing child care, access to medical services, acquiring transportation, increasing job skills, financial literacy training, developing life skills, and access to community resources.


The following are program components:

Intake and Assessment Process- Identifying and removing obstacles that hinder the client's ability to be self-sufficient (case management, free laundry services, meals and food boxes when needed, voicemail services, mail services, a safe place to reside, and connection to community resources).

Intensive Case management Services- Establish short and long term goals towards self-sufficiency through earning a living wage, financial literacy training, and life skills training.

Transportation- Provide transportation to and from evening classes, daycares and work.

Education and Training- This is key to increasing the client's ability to increase earnings. When this is an identified need, we will assist the client in enrollment and provide transportation to and from GED and college sites. In addition, we will provide educational related expenses such as tuition, testing, supplies and books.

Follow Up-90 day and extended follow-up upon the client's completion of their goals and individualized plan (home visits, phone contacts, client surveys, additional service referrals, unique opportunity through connection with the church).

Collaboration- Connecting clients to other social service agencies in the community in order to provide a comprehensive Individual Action Plan and support services focused on the individualized needs of the client.
