How to Register for the Angel Tree Program

The Salvation Army Angel Tree Program 2012

Serving children newborn to 12 years of age

In order to register, you must bring all of the following items with you.

1. State Issued Photo ID only and/or State Issued Drivers License for all applicants.

2. Official Proof of Address. (must be within the past month; phone bill, electric bill, tax return, lease, etc.)..Please note that if more than one family lives in the same house, all members of the household need to be present and sign up together.

3. Official ID showing age of children. (birth certificate, public housing lease, school record, statement from DHS listing children’s names, DOB and SSN etc.). (Those who are pregnant need to present documentation from physician stating their due date is before December 31, 2012)

4. Official Proof of Income. (must be recent, within the past month paycheck stub, statement from DHS showing what is received via Food Stamps and Families First, Workers Comp, Unemployment, Child Support, 2012 Income tax statement, etc. If paid in cash, must have proof on official letterhead)



WE SERVE ALL OF THE ZIP CODES IN DAVIDSON COUNTY EXCEPT: 37208, 37206, and 37203 North of Division St.

 

WE SERVE MOST OF THE 37209 ZIP CODE, MUST CALL TO VERIFY

WE SERVE ALL OF THE ZIP CODES WITHIN THE FOLLOWING COUNTIES:  SUMNER, WILLIAMSON, HICKMAN, DICKSON AND CHEATHAM

 

Angel Tree registration, October 4th-24th:

Monday-Friday

9:00 a.m.-12:00 p.m. and 1:30 p.m.-4:00 p.m.

 

 Thursday evenings from 5:30 p.m.-7:00 p.m.

Location – 3710 Vulcan Dr. (off of Allied Drive near the Nashville Zoo)



Senior Citizen Guidelines

In order to register for the Angel Tree Program as a Senior Angel, you must be at least
65 years of age or older and present the following information:


• State Issued Photo ID only and/or State Issued Drivers License for all applicants.

• Official Proof of Address. (must be within the past month; phone bill, electric bill, tax return, lease, etc

• Official Proof of Income. (must be recent, within the past month paycheck, award letter, 2012 Income tax statement.. If paid in cash, must have proof on official letterhead)





Individuals over 12 years of age and under 65 yrs of age with Disability Guidelines

You may register with The Salvation Army during the 2012 Angel Tree season for a 
Food Card only.
In order to register, you must bring the following items with you.

• State Issued Photo ID only and/or State Issued Drivers License for all applicants.

• Official Proof of Address. (must be within the past month; phone bill, electric bill, tax return, lease, etc

• Official Proof of Income. (must be recent, within the past month paycheck, award letter, 2012 Income tax statement, SSI letter.. If paid in cash, must have proof on official letterhead)

• Must present documentation stating there is a disability. This can be a recent SSI letter or a statement from your physician
